  3. Music Certification/Proof of License-may show on phone at coaches’ check in or email in advance

  4. WAIVER Forms for all athletes & coaches: organized and labeled by team: turned in at coaches’ check-in OR Google Drive Folder from your gym ( see below)

  5. Team Rosters- can be emailed or turned in at coaches’ check-in.

  6. Coaches & Personnel List; turned in at coaches’ check-in.

  7. Photo ID for all coaches and adult personnel ; shown at coaches’ check in

  8. Background check clearance required from all coaches and adult personnel from any accredited youth sports organization (WASF, USAG, USASF, AAU or fingerprint clearance card) .

  11. All programs must have valid insurance. Copy of COI may be required for 1st time programs (exception: school teams)

This is the ONLY way to submit forms electronically

  • Send this link to each of your families 

  • This is a fillable version.

  • once completed, parents will do a File> Save AS> and name the document in this format : GYM NAME_TEAM NAME _ ATHLETE LAST NAME_ATHLETE FIRST INTIAL OF THEIR FIRST NAME

  • Parents email you their form (do NOT send to VOS) 

  • Once you have collected all the forms for each athlete and verified you have all of them, save them into a Google Drive folder. 

  • Title the Google Drive Folder WAIVERS_PROGRAM NAME 

  • Share the Google Drive Folder and provide VIEWER access to: VOScheer@gmail.com

  • Send an email to confirm once you have created and shared the folder.

  • Wait for an email  CONFIRMATION of receipt from voscheer. If you do not receive a confirmation of receipt, please follow up to confirm the drive access.
